Who can file Form-8A

A person whose name is already included in the current electoral roll can file Form 8A. Applying for change of Address within the same Assembly Constituency that is the applicant is already a voter and has shifted his residence within the same  Assembly Constituency.

When Form-8A can be filed

1. The application can be filed after draft publication of electoral roll of the constituency for transposition of entry relating to one’s own self or someone else within the same constituency. The application is to be filed within the specific days provided for the purpose.

2. Only one copy of the application is to be filed.
3. Application for transposition of name within the constituency can also be filed throughout the year even when the revision programme is not going on. During non-revision period, application must be filed in duplicate.

Where to file Form-8A

1. During revision period, the application can be filed at the designated locations where the draft electoral roll is displayed as well as the offices of the Electoral Registration Officer and Assistant Electoral Registration Officer of the constituency.

2. During other period of the year when revision programme is not going on, the application can be filed only with the Electoral Registration Officer. The application should be addressed to the ERO of the constituency in which transposition of the relevant entry is required.

How to Fill the Form-8A

1. Enter the name of the constituency should be mentioned in the blank space.

2. Enter details of the person whose entry is to be transposed
Step 1 : Enter Name in English
Step 2 : Enter Surname (If Any)
Step 3 : Enter Name in Tamil
Step 4 : Enter Part number of electoral roll in which his/her name is included/
Step 5 : Enter His/her serial number in that part
Step 6 : Enter Elector’s Photo Identity Card number (if issued)
Step 7 : Enter Father’s/ Mother’s/ Husband’s Name in English
Step 8 : Enter Surname (if any)
Step 9 : Enter Father’s/Mother’s/ Husband’s Name in Tamil

3. Particulars of present place of ordinary residence
4. Enter Period of continuous residence at the above address on the date of application
5. Enter Part number to which the entry has to be transposed
6. Details of applicant
7. Please give your mobile number/Email I.D (optional)

    • ** For issue of ‘Duplicate’ (except in lost / misplaced cases) or ‘Replacement’ cards applicant should submit Xerox copy of original card with application.
      ** Claimants should surrender their original EPIC while collecting replacement EPIC.
      ** Officials will not hand over Duplicate card/ Replacement EPIC unless claimant surrenders his original (except in case of lost/ misplaced cases)
      ** At the same time submission of original will not be insisted upon in advance, i.e., till the new EPIC is ready for delivery to the person.

    • In case of loss, mutilation or poor quality of photo, the person will have to apply in person with photo in Form 001C along with proof of residence and remit the fee prescribed (Rs. 15/- ). in Treasury/ Sub-Treasury or in cash at the EPIC centre for which a receipt will be given by the election officials.
