Instructions

1. Register yourself in the citizens portal to get your username and password for your online submission by clicking on “New user? REGISTER”
2. To create your account click on Register.
3. Provide your valid e-mail Id and Mobile No. to complete registration.
4. After successful registration your user name and password will be sent to your e-mail and mobile phone.
5. You may now log in with your User id and password to submit your applications.

Form 7 Application Registration

Step 1 : After Login to your account click on the Form7 button.

Step 2 : They will be redirect to the Select AC Page where Citizen has to select the District Pin code, Locality and the Assembly Constituency

Step 3 : Fill the application details in the portal and Click on submit button to SUBMIT the application.

Step 4 : It’ll show a pop up message that “Do You Wish to Submit?” Click on YES button and it’ll proceed further.

Step 5 : Once you click on submit button Form 7 Acknowledge page will appear where you can find out the submitted applications.

Step 6 : Further click on the Generate Acknowledgement button for generating the submitted form 7 application.

Print Application

Your submitted application will displayed, You can take Print out of the same and sign and send the ERO/AERO Copy with the supporting documents to the Postal Address given in the ERO/AERO Copy.

The Status of your application wil be knowing through the link Application Status by entering the application No which is printed in the acknowledgement copy.

1. I will be attaining the age of 18 on 1.1.2002. What proof do I need to show to get enrolled?
You can submit the proof of Date of Birth from the authorized agency (Passport, Metric certificate, Date of Birth certificate etc.)

2. When I fill up Form 6, should I attach proof of my residence with Form 6?
It is not necessary but if you attach a proof of residence, this will help early verification of the particulars given by you.

3. When I fill up Form 4, should I attach proof of my residence with Form 4?
The special campaign are run for preparation of I card from time to time. The schedule & designated locations are published through News papers and are available on website. Icard are prepared on continuous basis in the office of Jt. CEO/DC of your area.

4. I have lost my old I Card. How can I get a new I Card?
You can deposit a copy of FIR lodged at the Police Station. You will get a new I Card after deposit Rs. 25. The dates for issuing I Cards are published in leading Newspapers, in Jt. CEO/DC office then cards are made through out the year.

  1. Is original voter ID to be attached along with From No -7 for deletion OR a Xerox copy of ID enough?
