Delivery Options
1. I have lost/misplaced my EPIC.

Visit the official website and download the form from the website, then fill the form with the required details

Issue of Duplicate EPIC
In case of loss, mutilation or poor quality of photo, the person will have to apply in person with photo in Form 001C along with proof of residence and remit the fee prescribed (Rs. 15/- ). in Treasury/ Sub-Treasury or in cash at the EPIC centre for which a receipt will be given by the election officials. EPIC will be given on collection of the prescribed amount (Rs.15/-)
Although EPIC remains valid in case of change of address (since it is used only for identification only), an elector, in case of change of address, may apply for new EPIC with new address for other purposes like when applying for passport etc. If an elector is already registered in roll in his new address, but change in address has to be made only in EPIC, then he has to apply in Form 001C along with details as in electoral roll. EPIC with change of address will be issued with same number.
Request for wallet-friendly EPIC will be dealt with on the same basis as those for change of address cases. The person has to apply in Form 001C.
Form 001C to be used in the above cases has two boxes for affixing 2 passport size photographs – 1 duly signed over photo and 1 photo without signature.
Such persons have to remit a sum of Rs.15/- either in cash at EPIC centre or in Treasury / Sub-Treasury through challan. The duplicate EPIC will be inscribed with the word ‘Duplicate’. New EPIC will have the same number as the original one.
Note :
** For issue of ‘Duplicate’ (except in lost / misplaced cases) or ‘Replacement’ cards applicant should submit Xerox copy of original card with application.
** Claimants should surrender their original EPIC while collecting replacement EPIC.
Officials will not hand over Duplicate card/ Replacement EPIC unless claimant surrenders his original (except in case of lost/ misplaced cases)
** At the same time submission of original will not be insisted upon in advance, i.e., till the new EPIC is ready for delivery to the person.
